Upload Forms

After a Form has been created and mapped, it can be uploaded to the System.

  1. From Libraries on the left Dashboard menu, select Forms in the Master Tables panel.

  1. Select Add.
  2. Form ID: A shortened title for the document. This name is used to link your document to Appointment Types of the form group APT.
  3. Form Name: The title of the document. 
  4. Form Group: Repeat this field for each location the form will be used.
  • APT: Form will be available in the Appointment menu.
  • CHG: Form will be available in the Encounter Dashboard
  • PAT: Form will be available in the Patient Dashboard.
  1. Template File Name: Enter the same content used in the Form ID field. The same shortened title is also used when linking the form to Appointment Event Types in the Charge Ticket field.
  2. Upload: Select Choose File and select the PDF from your computer.
  3. Select Save (F2).

If you wish to batch Print the Form from the Scheduling Dashboard, Link the Form to Appointment Types and Resources.

Otherwise, Form setup is complete after the Form has been uploaded. The Form can now be printed.
