ABN Form

What is an ABN Form?

  • An Advance Beneficiary Notice of Non-coverage (ABN) is a form that informs Original Medicare Patients that Medicare might not cover a specific service or item.

What are the fields that are available for mapping forms?


Why are the mapped form fields not generating/printing?

  • Has the form been uploaded to each necessary PM location? The form can only be generated/printed from an uploaded location.
  • Libraries > Forms > Add
  • PAT: Patient Dashboard
  • APT: Calendar
  • Don't forget to link the Appointment Types and/or Resources.
  • CHG: Encounter Dashboard
  • Ensure that each mapped form field has Visible and Print selected in the Text Field Properties. These fields are selected in the PDF Converter.
  • Read Only can be selected if these fields will never be modified in the PM.
  • Required can be selected to force the User to fill in the selected field.
  • Ensure that the correct form field mappings are being used based on the information that you are trying to generate.
  • Libraries > Forms > Add > Print List of Fields

Form Text Size

Why is the text the incorrect size?

  • The Text Field Properties inside of the PDF Converter determine the size of the text.
  • Auto-Size may be selected when the information is not fitting in the form field appropriately.
  • It may also be beneficial to update the size of the form field.

Why isn't the form uploading to the PM?

Insurance

Why isn't the Insurance populating on the form?

  • This information applies to Workers Comp forms.
  • When printing the form from the Appointment Menu/Calendar and a Case is attached, the Insurance will generate appropriately. If a Case is not attached, the Insurance will not be generated.
  • When printing from the Patient Dashboard, the Insurance must be the Default Insurance or it will not generate.

Why is the wrong Superbill generating?

  • If there is a Superbill attached to the Resource, it will supersede any Superbill set for the Appointment Type.

Why isn't a Superbill being generated?

  • Ensure that the form is uploaded to Appointment Types or Resources.
  • The form must be uploaded and linked to each Appointment Type that it will be used for.
  • The Superbill ID must match the Form ID.
