Create and Edit Forms

A form can be a superbill, a face sheet, or any other form.

You can create and maintain forms by purchasing and using a PDF Creator tool. 

  • We recommend using Nuance PDF CreatorPro, but any software that allows you to edit or write a PDF will work.

  • Find the instructions online for whichever tool you choose.

    • Youtube would be a good place to start.

  • Create the form, which is typically done in MS Excel or Word.

  • Save the form as a PDF document (PDF only).

  • Open the PDF in Nuance PDF or another PDF Editor.

Map Form Fields

System Fields can be mapped to populate uploaded Forms.

Forms can be printed from the Appointment screen, the Encounter Dashboard, and the Patient Dashboard.
  1. Superbills may also be referred to as Encounter Forms, Fee Slips, or Tickets.

System Setting for Printing Order of Superbills and Forms

  1. Admin > System Settings > Scheduling > TICKET_SORT.
    1. This setting determines in what order Charge tickets/superbills will print.
      1. Time (and ---): Tickets will print from earliest to latest appointment.
      2. Resource, Time: Tickets will print grouped by Resource, then by Appointment time for each Resource.
      3. Last Name: Tickets will print alphabetically based on the Patient's last name.
      4. Resource, Last Name: Tickets will print grouped by Resource, then alphabetically by Patient's last name for each Resource.
    1. Note that tickets are printed with a single click from the Calendar. They will print for the Resource/Facility options that are selected on the Calendar at the time of printing, using the group/sort option selected here.
