Upload Forms

After a Form has been created and mapped, it can be uploaded to the System.

  1. From Libraries on the left Dashboard menu, select Forms in the Parent Tables panel.
  2. Select Add.


Form Panel (All Forms)

  1. Form Type: Determine where this form will be available (Patients, Appointments, or Charges).
  2. Template File: Select Choose File and select the PDF from your computer.
  3. Enter a meaningful TItle and Description.
  4. POPS Form: Check this box to make this form available for completion in POPS.
  5. Patient Facing Description: If you want Patients in POPS to see a different name for the Form, enter it here.
  6. Enter an Effective/Expiration date.


Criteria Panel (POPS Only)

  1. Signature Valid For: Enter a number of Days, Months, or Years.
  2. Patient Age >=: Enter the minimum age of a Patient that should be receiving these Forms.
  3. Patient Age <=: Enter the maximum age of a Patient that should be receiving these Forms.
  4. Select the relevant Appointment criteria (Appointment Type, Appointment Resource, Facility, and Insurance).
  5. Select Save [F2].

Next Steps

Map the Form with dynamic fields to allow data to automatically populate the Form from the PM, or to allow Patients to complete Forms through POPS.

If you wish to batch Print the Form from the Scheduling Dashboard, Link the Form to Appointment Types and Resources.
