Patient Dashboard Notes Overview

Notes Overview
Users can manually add Notes on the Patient Dashboard, and some Notes that are added in other areas in the System can also be seen here. The System also creates Notes when certain actions are performed.

Add a Note

Add a Pop-Up and/or Header Note
From the Patient Dashboard Notes panel:
  1. Select the Add link or the Notes link > Add button.

                       

  1. Select or Create a Category

    1. Select an existing Category from the dropdown.

    2. Or, select Manage to add a Custom Category.

  2. Enter the Note Text

    1. Enter a clear, concise description of the Note.

  1. Set Alert Options

    1. Pop-Up Alert: Displays a pop-up when the Patient is opened.

    2. Header Alert: Shows the alert at the top of the Dashboard or Screen.

    3. Both: Use both options.

  2. Specify Effective and Expiration Dates (optional)

    1. Use these Dates if the Note should only apply to a certain time frame.
      Add Dates for when this Note should be Effective and when it should Expire, if applicable. (Optional)

  3. Select display locations.

    • Select the Check/Uncheck link once to check all the boxes.

    • Select twice to clear all the boxes.

    • Select the boxes for where you want the Note to appear, such as Encounters, Patient Dashboard, Encounter Dashboard, etc.

  1. Select Applicable Encounters (optional)

    • Indicate which Encounters to apply this Note.

    • If specific Encounters are selected here, Encounters must be checked in the Alert Options panel.

    • Conversely, if the Encounters checkbox is selected in the Alert Options panel, and there are not any specific Encounters selected in the Apply Note to Encounters panel, the Note will display on all Encounters.

  2. Select Save [F2].

            

Manage Note Alerts: Snooze, Remove, Archive

Snooze a Note Alert

Temporarily hide a Pop-Up Alert

  1. When the Pop-up appears, select Snooze.

  2. Select a Snooze duration (The alert will reappear after the selected time.)

    1. Default Times

      • 5 Min (Default)

      • 10 Min

      • 15 Min

    • Use the System Setting to add a Custom Time.

  3. Select Save [F2].

  1. Validation Warning: "You are about to Remove the Alert for this Note."

    • This does not delete the Note. It only removes it from showing in the Alerts.

  2. Other Validation Warnings:

    • Other Note Alerts may exist, such as "Your are about to Snooze this Note for 5 minutes."

  3. Select Save & Continue.

    • If Save & Continue is selected, on all Note Alerts listed in the Validation Warning, the actions will be performed.



Remove a Note Alert

An Alert can be removed without deleting the Note from the Notes panel.


On the Note Alert Pop-Up:
  1. Select Remove Alert.

  2. Other Note Alerts may exist.

    • If they are left as Snooze and 5 min, they will be snoozed for 5 minutes.

  3. Select Save [F2].

  1. Validation Warning: "You are about to Remove the Alert for this Note."

  2. There may be other Note Alerts on the Validation Warning.

  3. Select Save & Continue.

    • All actions on the Validation Warning will be performed.

      • The Alert marked as Remove Alert will not be in the Alert Pop-up any longer.

      • The other two Note Alerts will not display for the next 5 minutes.

      • They can be viewed and edited in the Notes Panel on the Patient Dashboard.


Archive/Unarchive a Note

A Note can be Archived so that it no longer shows in the active Note Panel.

  1. It can still be viewed using the Plus icon, or
  2. Select the Notes link, where it can be filtered to include or see only the archived Notes.

Archive a Note Using the Alert Pop-up or Alert Header

  1. Select Archive Note.

  2. Other Note Alerts may exist.

    • If they are left as Snooze and 5 min, the Note Alerts will be snoozed for 5 minutes.

  3. Select Save [F2].

  1. Validation Warning: "You are about to Archive this Note."

  2. There may be other Note Alerts on the Validation Warning.

  3. Select Save & Continue.

  • All actions on the Validation Warning will be performed.

  1. The Alert marked as Archive Note will not be in the Alert Pop-up any longer.

    • The other two Note Alerts will not display for the next 5 minutes.

      • They can be viewed and edited in the Notes Panel on the Patient Dashboard.

  1. The Note will be removed from the Active Note List.

  1. The Note can still be viewed if the Plus icon is selected in the Notes panel.


Archive a Note using the Active Note List

  1. In the Notes panel, select the Archive link.

  1. On the Pop-up, select Yes for "You requested to Archive this note. Would you like to continue?

  1. The Note will no longer be in the active Notes list.

  1. The Archived Notes can be viewed using the Plus icon.

Unarchive a Note 

  1. Select the Notes link in the Notes panel.

  2. Select the Unarchive link.

  1. Select Yes for "You requested to Unarchive this note. Would you like to continue?"

 View Notes

View the Notes List in the Notes Panel
The most recent active Notes are displayed in the Notes Panel.

Columns Represented on the Notes List

  1. Modify Icon: Select to make changes to the Note.

  2. Encounters: Encounters where the Note is located.

  3. Note: Description or action that created the Note.

  4. Category: This is the Category that was selected when creating the Note.

    • This column is blank on an Auto-Note

  5. Added: Date the Note was added.

  6. By: User who added the Note or performed an Action that created an Auto-Note

  7. Last Modified: The date when the Note was modification.

  8. By: Who made the modification

  9. Alerts: Type of Note

    • P: Pop-Up

    • H: Header or Both

    • P/H: Both Pop-up and Header

  10. Archive: The Note can be Archived or Unarchived using the link in this column.

View All Notes
Use the Plus icon to View all Notes including Archived and Unarchived Notes
  1. Select the Plus Icon to view all Notes.

  2. To view the Encounters Notes as well, select the Include Encounter Notes checkbox.

  1. Notes can be Archived and Unarchived using the links in the last column.


Download/Print/Save a Note List

  1. Download as an Excel document
  2. Download and Print a PDF document
  3. Save a Note List
  4. Clear Current Filters
  5. Temporarily Hold selected Filters
      

View Filtered Notes
By default, all active Notes will display in the list.
  1. Select the Notes link in the Notes panel.

  1. Use the Filters to view specific Notes, such as Archived Notes.

  • You may need to select Show to see the Filters.

  1. Filters

    • All Patient Search: Check the box to see all Patient Notes

      • This is used when looking for a specific Category, User, etc.

    • Note: A word or words can be entered that may be in the Note.

    • Category: All or select a Category

    • Added by: All or select a User

    • Modified by: All or select a User

    • Added From/Added To: Range of Dates that the Note was added.

    • Modified From/Modified To: Range of Dates that the Note was modified

    • Alert: All, Pop-up, Header, Both, None

    • Encounter: Enter an Encounter number

    • Effective/Expiration: An Effective Date and/or Expiration Date can be entered to narrow your search dates.

    • Archived: Exclude, Include, Only


  1. Add a Note: Use the Add button to create a new Note.

  2. Delete a Note: Use the Delete button to permanently delete a Note.

    1. Deleting a historical Note is not recommended.

    2. Alternatively, the Note can be Archived or a Expiration Date can be entered on the Note.


Print/Download/Save Notes

 Print Filtered Patient Notes from the Patient Dashboard

  1. In the Notes Panel on the Patient Dashboard, select the Notes link.

  2. Select the PDF icon .

  1. Select your Print icon (Yours may look different than the screenshot below.)

  2. Follow your printer's dialogue instructions, which may be different than below.


Download a Notes List

A PDF or CSV document can be downloaded to your computer.
  1. Select the Excel (csv) or PDF icon.
  2. PDF Download
      
  1. CSV Download
    1. Save as: Save as an Excel Document before making any changes.
      1. The document can be renamed.
      

      
  1. After saving, the document can be modified and the modifications can be saved.
      


Print Patient Notes from the Custom Query Report

Patient Notes can also be printed using the Custom Patient Query Report for a specific Patient for specific dates.

  • A Date Range can be used to pull specific notes for a specific date(s).

  • The Patient ID can be used to pull a specific Patient.

  • Select Column, Patient > Notes

  • Use the PDF   icon to export.

Delete a Note

Delete a Note

It is not recommended to delete Notes unless it was entered by mistake.

  1. The Notes serve as the history of what has occurred on this Patient.
  2. Archive or Expire the Note instead.

Delete

  1. Select the Note to highlight it.

  2. Select the Delete button.

  3. Enter a Reason (mandatory).

  4. Select the Delete button.
