Create a Patient Receipt/Refund Receipt Form

Using the Form links below, Patient Receipt and Refund Receipt Forms can be created and uploaded into the PM System.

Map and Create a Patient Receipt Form

  • Use the System's Default Patient Receipt to create a new Receipt.
  • Libraries > Forms > Add
  1. Form Type: Receipt
  2. Template File: Select the Receipt.
  3. Enter a Title and Description.
  4. Select Save [F2].

Map and Upload a Refund Receipt Form

Notes

NOTE: A Patient Refund Receipt is not the same as a Patient Receipt Form.

These must be set up separately and are handled differently.

  • Use the System's Default Refund Receipt Form to create a new Refund Receipt.
  • Libraries > Forms > Add
  1. Form Type: Receipt
  2. Template File: Select the Refund Receipt.
  3. Enter a Title and Description.
  4. Select Save [F2].
  • Map the Form to add the dynamic Patient Payment and Refund fields.
