Contains: Looks at any part of the value.
Example: A Patient has the last name Shoemaker.
The Last Name Contains = "Shoe" Filter will pull the Patient last name Shoemaker since "Shoe" is included in "Shoemaker".
Includes: Looks at the whole value.
Example: A Patient has the last name Shoemaker.
The Last Name Includes = "Shoe" Filter will not pull the Patient last name Shoemaker.
The Last Name Includes = "Shoemaker" Filter will pull the Patient last name Shoemaker since the entire value is included in the Filter value.
Method 2: Add the Filter Encounter - CPT List as a Column and create a comma separated list.
The font on these reports can only be changed if exported to CSV: Custom Queries, Collection Agency, Reference Batch, Interactive Aging, and Period End Reports cannot be changed unless you export the output to CSV.
The font can be changed on most other reports.
At the bottom of the filters screen, select the Font Size: S, M, L, XL, or L.
The Address is stored in Statement Profiles (Admin > Statement Profiles).
Select the Statement Profile ID and enter the Return Address.

All Custom Query Reports can be run by Race or Race ID: CEQ, CPQ, CTQ, CRQ, and CAQ.
Examples:
CPQ: Custom Patient Query (Under Demographics panel > Race; use Contains, not Equals)
CAQ, CEQ, CTQ, and CRQ
CEQ: Custom Encounter Query (Under Patient Panel > Race ID or Race; Using Contains, not Equals)
