Create a New Case

1. Type: Choose from the dropdown or choose Manage to create you own.
2. Description: Type the description of the new Case.

• Example: PCD REFERRAL LSF (Type of Referral plus Assigned Provider ID)

3. Insurance Profile: Choose the applicable Insurance Profile.

4. Assigned Provider: Use magnifying glass to choose Assigned Provider.
5. Referring provider: If applicable, use the magnifying glass to choose the referring provider.
6. Select the Add Authorization link.
7. Authorization Number: Enter the PCP Authorization number.
8. Effective Date: Enter if applicable.
9. Expiration Date: Enter if applicable.
10. Visits: Enter the number of allowed visits if applicable.
11. Select Save.
