• DOS: Date of Service
• Line: Line Item # that indicates the position of the Charge relative to other Charges on the Encounter
• Proc ID: Procedure Code where the tax was applied
• Units: Number of Units on the Charge.
• Fee: Charge amount.
• Fee Total: Charge Fee Total
• Total Tax
• Patient ID
• Patient Name
