Cases for PCP Authorization

Create a New Case

  1. Type: Choose from the dropdown or choose Manage to create you own.
  2. Description: Type the description of the new Case.
  • Example: PCD REFERRAL LSF (Type of Referral plus Assigned Provider ID)
  1. Insurance Profile: Choose the applicable Insurance Profile.
  1. Assigned Provider: Use magnifying glass to choose Assigned Provider.
  2. Referring provider: If applicable, use the magnifying glass to choose the referring provider.
  3. Select the Add Authorization link.
  4. Authorization Number: Enter the PCP Authorization number.
  5. Effective Date: Enter if applicable.
  6. Expiration Date: Enter if applicable.
  7. Visits: Enter the number of allowed visits if applicable.
  8. Select Save.
