CO16 Denial: Missing or Invalid Other Insurance Indicator (CFI) on a Secondary Claim

Denial

Secondary Denial: CO16 Missing/Incomplete/Invalid Other Insurance Indicator

  • On a secondary Claim, this means Loop 2320 either had an incorrect or missing Claim Filing Indicator (CFI) for that other subscriber.

Correct the CFI

Info
The CFI should be based on the actual type of Insurance coverage for the Product. Do not select a CFI based solely on the Payer name. If you are unsure which CFI is appropriate, verify the Patient's specific Insurance Product and coverage type before making a change.

When a secondary Claim is rejected because the Claim Filing Indicator (CFI) for the primary insurance is missing or incorrect, follow these steps:

  1. Identify the Insurance Product listed on the Patient's primary Insurance Policy.
  2. On the corresponding Insurance Library record, select Products.
  3. Select the Modify icon next to the Product.
  4. Update the (CFI) to accurately represent the type of coverage.
  • Note: This changes the CFI for this Product on all Claims.
  1. Select Save [F2].
  2. Return to the affected claim and change the claim status to Ready for Resubmission so the claim can be reprocessed with the corrected information.
