POPS Profile

The POPS Profile page provides a quick view of Patient information. 

  • On this page, Portal Users can Register a Patient or Modify a Patient.

Access a Profile

There are multiple ways that a Portal User can access their Profile.

  • In the User dropdown, select Profile.

  • On the Home Page, select View Profile.

  • On the Account Dashboard, select View Profile.

  1. Contact Information: Update the Preferred Name, Email, or Phone.

    • Select the Verify icon to confirm the contact point.

  1. Document Upload: Upload a Driver's License or Selfie (optional).

    • Successfully uploading a Driver's License will populate the relevant Demographic Information in the panel below.

  1. Demographics: Enter or revise the Demographic Information.

      
Notifications
  1.   Balance Notifications: Choose the preferences for Email, Text, Voice, or Paper Balance Notifications.

  2. Appointment Reminders: Choose the preferences for Email, Text, or Voice Appointment Reminders.

      
Forms
  1. Select Sign Form to open the Form.

  1. Complete all necessary fields.

    • To learn more about the available Fields, see Map Forms.

  1. Select Download to download a completed Form.
