There are many reasons why you may be getting this error.
- The Patient is no longer covered by this Insurance Plan.
- Resolution: Get new Insurance Information from the Patient.
- The Claim was sent to the wrong Payer.
- Resolution: Check Insurance Information to validate the Payer and Payer ID. Make corrections if necessary.
- Invalid/Missing/Incorrect Insured information.
- Submitted incorrect information:
- Name of Insured/Subscriber could be spelled incorrectly.
- The name on the Insurance card may not be the name you have in the System.
- The birthdate could be incorrect.
- Resolution: Correct if necessary.
- Invalid/Missing/Incorrect client/Patient information.
- Misspelled Name
- Wrong Birthdate
- Resolution: Make corrections.
