Who Made Changes to the Patient Data and When

You can view changes made to your data: displaying the user name, what was changed, and what it was changed to.

The Change Log is available from:

  • Patient Dashboard

  • Encounter Dashboard
  • Appointments screen

Admin Dashboard logs changes across a variety of screens). See Change Log from the Admin Dashboard below.

Additions, Deletions, and Changes are tracked for the following:

  • Patients
  • Appointments
  • Cases
  • Notes
  • Encounters
  • Procedure Lines
  • Receipts
  • Reference Batches
  • Users
  • System Settings
  • Master Tables and Code Tables are tracked in Libraries

Change Log from the Admin Dashboard

The Change Log can be accessed from the Admin Dashboard under Admin Data.


  • EntityThis is the Record type you are searching for. For example, you would select Patient to find all changes made to Patient information.
  • ID: If there is a specific record ID you are looking for, enter it here. For example, if you chose Patient as your Entity, you could enter a Patient ID here to only look at that specific Patient. 
  • Field: This filter allows you to choose a specific field that is part of the Entity record you chose.
  • Date From/To: The Date From and Date To fields allow you to find changes that were made within a specific date range.
  • User: This filter allows you to find changes made by a specific user.
